Bidder are requested to furnish the following:
- Bidders should submit copies of valid and renewed trade license for the year 2012 E.C, VAT registration certificate, and Tax payer’s registration certificate & Certificate of Competence for Medical from Ethiopian Food and Drug Authority
- Bidders should also present testimonials as evidence of previous experience in similar large purchase activities of the same kind of specification from eligible past customer /user
- Bidders should provide wholesale Certificate or Manufacturers’ Authorization form.
- Completed bids should be submitted with sealed envelope to Cuso International office. The deadline for submission of bids is August 19,2020 at 5:00 A.M Ethiopia Time
- Quantity of the supplies needed may be higher or lower than those indicated on the table above based on the fair price provided by the bidders and Cuso International Budget
- Failure to comply with any of the conditions indicated in this bid will result in automatic rejection without any consideration.
- Please, note that public bid opening is not applicable for this invitation to bid.
